Dean, Dorton & Ford asks that all organizations requesting financial support from us complete this questionnaire. We ask that your request be submitted at least two months in advance for proper consideration. Completion of this form does not guarantee that Dean, Dorton & Ford will be able to fulfill the request.

  • Formal request on organization letterhead
  • A copy of the IRS designation letter
  • List of current corporate sponsors (where available)
  • Promotional materials supporting the project
